Q: What modality and protocol is best for a patient with a cerebrovascular accident (CVA) and shoulder subluxation?
A: Patterned electrical neuromuscular stimulation (PENS) subluxation protocol.

Q: Where should I place the electrodes for interferential current (IFC) to the low back, and does the polarity matter?
A: Place four electrodes surrounding the area, cross the channels, and use polarity to target the sweeping of the vector
over the painful area.

Q: My patient had a total knee arthroplasty (TKA) and a high level of pain with profound weakness. I used IFC with good result for pain reduction. How do I progress my patient?
A: Lower extremity triphasic PENS to the quadriceps/hamstrings alternated with anterior tibialis/gastrocnemius protocols
would address disuse atrophy and weakness. This can be progressed to medium frequency alternating current (MFAC)
to the quadricep alternated with MFAC to the anterior tibialis.

Q: Is treatment with modalities contraindicated for a patient with pain who has a pacemaker?
A: Both electrical stimulation and diathermy are contraindicated. Ultrasound (US) can be used for pain, but not over the
pacemaker region.

Q: I am trying to set up head and neck PENS for my patient with a CVA and dysphagia. Where do I put the black (-) and red (+)?
A: The red (+) goes on the face and the black (-) goes on the back of the neck.
